(36) Acute lumbar sprain

Acute lumbar sprain is a condition caused by acute injury to the ligaments connecting the lumbar spine and the muscles on both sides of the spine, mainly low back pain and immobility. The spine itself and surrounding soft tissues are susceptible to direct or indirect force damage. Traditional Chinese medicine believes that the main pathogenesis of this disease is the damage to the waist meridians, qi stagnation and blood stasis caused by falls and frustrations. Patients generally have a significant history of sprains. Immediately after severe injury, symptoms of low back pain, inconvenience, difficulty sitting and lying down, and even inability to take care of oneself, aggravated symptoms when coughing and deep breathing; In mild cases, the symptoms are not obvious at the time, but after a few hours or the next day, the symptoms gradually develop lumbar dysfunction, which can lead to pain by pulling on the injured muscles when moving in all directions.

(1) The patient is placed in the prone position, and the surgeon rubs the psoas muscles on both sides of the patient with the palms of both hands for 3~5 minutes to relax the tense and spasmodic lumbar muscles, as shown in Figure (1).

(2) The surgeon pinpoints the pain point of the patient's waist, and rubs the Ah Shi acupoint with the thumb of the left hand for 3~5 minutes on the pain point, and the strength should be gentle and gentle, not too heavy, see Figure (2).

(3) The surgeon uses the thumbs of both hands to slightly press and rub the patient's acupuncture points such as Weizhong, Weiyang, and Chengshan for 1~3 minutes per acupoint, see Figure (3).

(4) The surgeon rubs the psoas muscles on both sides of the patient with the palms of both hands or the thenar part for 1~3 minutes, and the local skin feels warm, as shown in this figure (4).

2. Precautions

(1) Pain in the lower back will occur in many diseases in clinical practice, and treatment should be carried out after a clear diagnosis.

(2) Patients with severe sprains should not be treated with hot compresses within 24 hours, nor can they be treated with excessive manual treatment.

(3) In the acute stage, the patient should mainly rest, walk on the ground appropriately, and move the waist.

(4) During the attack, the stool should be kept smooth to avoid aggravation of the condition due to constipation.
