(33) Piriformis syndrome

Piriformis syndrome can cause sciatica, but no lower back pain is present. Piriformis muscle entrapment of the sciatic nerve, local adhesions, congenital anomalies, tumors, etc., can all lead to piriformis syndrome.

The clinical disease is mostly unilateral, with hip pain and sciatica on the affected side. Some patients may have intermittent claudication, numbness on the affected side, pain points in the buttocks pressed, and radiating pain in the lower extremities on the affected side. Pain occurs when the straight leg is raised less than 60° and is less painful when it is greater than 60°. Pain in the lower extremity on the affected side worsens when the waist is bent upright; When the waist is stretched upright, the pain in the lower extremity on the affected side is reduced.

(1) The patient is placed in the prone position, the surgeon is located on the affected side of the patient, and the leg is rubbed on the affected part of the patient for 3~5 minutes with the palm root kneading method to relieve local pain, as shown in this figure (1).

(2) The surgeon uses the elbow tip kneading method to repeatedly operate on the piriformis muscle on the affected side of the patient for 3~5 minutes, with appropriate strength, to the extent that the patient can accept, see this figure (2).

(3) The surgeon uses the thumbs of both hands to overlap and rub the affected area A is acupoint and pain point (usually on the inner upper part of the buttocks), and repeatedly press and knead for 3~5 minutes, see this figure (3).

(4) Immediately after the above method, the surgeon then uses his thumb to rub the affected limb Fengshi, Yanglingquan, Weiyang, and Chengshan acupoints for 3~5 minutes, and the force can be slightly heavier, see this figure (4).

2. Precautions

(1) The symptoms of this disease are similar to other clinical locomotor system diseases, and the symptomatic treatment should be clarified before treatment.

(2) Generally, after more than 3~5 times of treatment, the symptoms will gradually reduce or improve. If it does not decrease or worsens, you should go to the hospital for further examination and treatment.

(3) During the attack and treatment, heavy physical labor should be avoided, especially the weight bearing of the waist and lower limbs.

(4) Self-percussion therapy should be gentle and short, so as not to cause local muscle and nerve and other tissue damage.
